AUTHORIZATION:

The following were selected as a candidate(s) for ordination to the Gospel Ministry.
Authorization was given for an ORDINATION COUNCIL to meet _____/_____/__  __  for the purpose of questioning the candidate(s) being considered for the Gospel Ministry. The Council to consist of all Ordained men present on the appointed day of examination.
